Identify the page URL and exact statement, explain what appears wrong or outdated, and include a primary or otherwise reliable source when available. Do not share sensitive medical or personal information.
How updates are shown
Minor spelling and formatting changes may be made silently. Material factual corrections are noted on the affected page with the date and a short explanation.
